Medicare’s New Payment Model is Built for AI-And Most of the Tech World Has No Idea

The United States Medicare programme is rolling out a major new payment system designed specifically to support artificial intelligence and technology-driven care. Many in the tech industry are yet to fully grasp its significance.

The model, known as ACCESS (Advancing Chronic Care with Effective, Scalable Solutions), started in 2026. It moves away from the traditional fee-for-service approach, where doctors and providers are paid for each visit or procedure. Instead, it pays organisations based on actual health outcomes for patients.

Under this system, participating care organisations receive predictable monthly payments to manage chronic conditions such as diabetes, hypertension, chronic pain, depression, and anxiety. They earn the full payment only when patients show real improvement for example, better blood pressure control or reduced pain levels.

This outcome-based structure strongly favours companies using AI tools. Artificial intelligence can help with early diagnosis, continuous remote monitoring, personalised treatment plans, and predictive analytics all of which help achieve better patient results.

The model is voluntary and runs for 10 years. It is expected to create new opportunities for health tech companies that combine AI, remote devices, and human care teams. One early participant, Pair Team, has already been accepted into the programme to expand AI-enabled care for Medicare beneficiaries.

For millions of older Americans, this could mean more proactive and personalised care instead of waiting for problems to get worse. Supporters say it encourages innovation while controlling costs, as payments are tied directly to results.

However, some experts worry about potential risks, such as over-reliance on technology or unequal access for patients who are less comfortable with digital tools.

The ACCESS model is one of the clearest signs yet that America’s largest healthcare payer is preparing for a future where AI plays a central role in everyday medical care. While many Silicon Valley companies remain focused on general AI applications, healthcare specialists believe this quiet shift by Medicare could create massive opportunities for those who understand the new rules.

As the model expands, it may influence how healthcare is paid for not just in the US, but in other countries watching closely for effective ways to manage ageing populations and rising medical costs.
